The outreach program helps to link individuals with appropriate services and increase general community awareness of the services offered by the Agency and other service organizations.

Outreach services include:

• Assistance with completing Medicaid/MIChild/Healthy Michigan Plan applications, as well as assistance in signing up for public assistance programs offered through the Michigan Department of Human Services.

• Community health fairs and events.

• Agency service presentation.
