This is a program provided directly by Region IIIC Area Agency on Aging.  It fulfills clients' wishes to remain in their own home by arranging in-home long-term care services when they are at risk or in need of nursing home placement.  The program is available to persons who are:

• 60 years of age and older

• Medically complex

• Currently a resident of a skilled or intermediate facility no longer requiring that level of care, but is unable to obtain the needed community services to remain home

• Currently at home, but considering a move to another location

• At risk or in need of nursing home placement

Some of the services arranged by Care Management include:

• Home Delivered Meals

• Personal Care

• Respite Care

• Light Housekeeping

• Personal Emergency Response Systems

• Medication Management and Assistance

• Referral to other appropriate agencies if Care Management is no longer able to meet the needs of the client
