Community Health Workers (CHWs) help bridge the gap between healthcare and the community by connecting individuals and families with the services, education, and resources they need to improve their health and well-being. At the BHSJ, our Community Health Workers provide personalized support and empower people to overcome barriers to care.
Community Health Workers (CHW)
Community Health Workers are available to assist residents in addressing the social determinants of health, such as housing, access to care, or food security, to improve individual health outcomes. Community Health Workers partner with local agencies to help meet individuals’ needs.
