File my weekly school reporting?

1. On the home page, under the Clinical Services tab, click on Communicable Disease Reporting Requiremetnts.

2. On the Communicable Disease Reporting Requirements page, scroll to the bottom to the Resources section.

3. Select the CD Reporting School and Daycare link appropriate for your county.
